Overview
This documents lists down the ACH return codes that are used when a transaction is returned by the bank or financial institution. Each code provides insight into why the transaction failed or was reversed.

Authorization Revoked |
Consumer has revoked authorization for this payment. |

Payment Stopped |
Payment was stopped by the account holder. |
|

Uncollected Funds |
Account has sufficient balance, but funds are unavailable. |

Customer Advises Not Authorized |
Customer claims they did not authorize the payment. |

Check Truncation Return |
Problem with an image-based check transaction. |

Branch Sold to Another DFI |
Branch/account no longer exists due to acquisition or sale. |

Invalid ACH Routing Number |
Routing number is incorrect or no longer active. |

Representative Payee Deceased |
The payee (acting on behalf of someone else) has died. |

Beneficiary Deceased |
The actual intended recipient of funds has passed away. |

Account Frozen |
Account has legal or regulatory hold. |

File Record Edit Criteria |
Record could not be processed due to formatting or data errors. |

The transaction date is invalid. |

Amount Field Error |
Non-numeric or improperly formatted amount. |

The Company ID does not match RDFI’s records. |

Invalid Individual ID Number |
The individual ID (SSN, TIN) is invalid. |

Credit Entry Refused by Receiver |
Receiver refused the funds—wrong amount, date, or reason. |

Duplicate Entry |
Entry appears to be a duplicate of a previous transaction. |

Addenda Error |
Addenda record is invalid or improperly formatted. |
